目的 探讨口腔种植修复中采用上颌窦内提升术对种植体成功率及牙周健康指标的影响。方法 选取2014年6月—2016年1月山东省青岛疗养院收治的上颌前磨牙或磨牙缺损实施口腔种植修复治疗的患者80例进行回顾性分析。根据治疗方法分为A组40例(采用上颌窦内提升术实施种植修复)、B组40例采用常规种植修复技术。对比两组术后1年、2年种植体留存率、术后不同时间点种植体周围骨吸收量、牙周菌斑指数、探诊深度、龈沟出血指数。结果 术后1个月、术后6个月和术后12个月A组患者的种植体周围骨吸收量显著的高于B组;术后24个月,A组和B组患者的种植体周围骨吸收量差异不具有统计学意义;术后6个月,A组患者的牙周菌斑指数、探诊深度、龈沟出血指数均显著的低于B组;术后1年,A组和B组患者的种植体留存率比较,差异不具有统计学意义;术后2年,A组患者的种植体留存率96.00%高于B组患者的83.02%。 结论 口腔种植修复中采用上颌窦内提升术有利于促进种植体周围骨吸收、提高种植体的留存率、降低种植修复对牙周健康的损害程度。 相似文献

During the normal healing process, an extraction site may lose significant bone volume, making implant placement problematic. Quantitative evaluations of the amount of bone maintained by socket preservation with various materials are limited. The objective of this study was to evaluate, both clinically and histologically, the extent of alveolar bone preservation by blood coagulum (BC) and the potential additional benefits of bone allograft material (AL) versus the state-of-the-art bovine bone mineral (BB), covered by a polyethylene glycol (PEG) barrier, in extraction socket grafting procedures. Adult patients (n = 32) with single-rooted teeth indicated for extraction were treated (45 sites). After atraumatic extraction, the sockets were filled with BC, AL, or BB and covered with a synthetic PEG barrier membrane. Changes in bone height and width were measured clinically and the amount of bone formed and residual graft particles were measured histologically after 6 months. Changes in ridge width at 6 months were ?1.5 mm for AL versus ?2.5 mm for BB and ?2.3 mm for BC. New bone formation amounted to 47.8%, 33.3%, and 28.2% at BC-, AL-, and BB-treated sites, respectively. Using AL with the PEG barrier preserved the ridge width at 6 months better than BB or BC and resulted in similar amounts of bone histologically to BB. 相似文献

External root resorption (ERR) affecting mandibular second molars (M2) may occur when the adjacent third molar (M3) is impacted in close proximity. This retrospective cohort study aimed to assess the presence, severity, and location of ERR on M2 due to M3 using cone beam computed tomography (CBCT) scans and to identify associated factors. The angle between the axis of M2 and M3 was measured. ERR on M2 was classified as absent, slight, moderate, or severe. The location of contact between M3 and M2, the size of the dental follicle, and patient demographic characteristics were recorded. A total of 433 patients with 640 M3 were included. A male predilection was found with regard to ERR (P = 0.0004). ERR was identified on 31.9% of M2 and was slight in 30.2%, moderate in 1.4%, and severe in 0.3% of cases. The presence of ERR was associated with direct contact between M2 and M3 (P < 0.0001), the angle between M2 and M3 (P < 0.0001), the inclination of M3 (P = 0.001), and the location of contact (P = 0.0005). This study showed ERR to be a frequent finding. ERR is associated with a mesioangular position of M3 in more than one third of cases, and a proximity ≤0.5 mm between M2 and M3 favours ERR. 相似文献

Milk remains one of the most frequently recommended solutions for storage of avulsed teeth because it can maintain cell viability and is easily accessible. However, some negative effects of milk on avulsed teeth have been reported, just as the effects of milk on the long‐term functions of cells are not clear. This study aimed to evaluate the effects of different types of milk on the viability, proliferation, and functions of periodontal ligament fibroblasts (PDLF)s in vitro. Human PDLFs were culture‐medium depleted for 5 min and stored in Hanks’ balanced salt solution (HBSS), whole cow's milk, low‐fat cow's milk, or almond milk for 1 h at 25°C. Cell viability and proliferation were assessed using MTT assays. Expression of the genes encoding type I collagen and its modifying enzymes were analyzed using real‐time PCR. Collagen matrix production was evaluated using Picrosirius red polarization. Our results showed the overall efficiency of low‐fat cow's milk in maintaining the viability and proliferation of PDLFs, and in enhancing the process of collagen production. Almond milk storage resulted in the highest rate of PDLF proliferation, and comparable collagen biosynthesis ability to the control. Therefore, besides low‐fat cow's milk, almond milk may potentially be an alternative tooth‐storage medium for PDLF preservation and PDL tissue regeneration. 相似文献

BackgroundDental trauma injuries are frequent in children and adolescents and can result in a sequela of future complications. Lateral luxation injuries are diagnosed when a tooth becomes displaced in a position other than axial and is often associated with alveolar bone fracture. Although the tooth is not immediately lost, pulp canal obliteration or pulpal necrosis can occur. The objective of this systematic review was to gather existing data on lateral luxation injuries to mature teeth to evaluate their overall prognosis and reported complications.Types of Studies ReviewedThe authors conducted a systematic search of the literature using MEDLINE, PubMed, Embase, and Cochrane databases in February 2019. They hand searched reference lists to identify additional literature. The authors included prospective and retrospective observational studies in the search. They screened a total of 291 articles, downloaded 28 articles, and included 4 articles in the study.ResultsThe most frequent complication reported for mature teeth with lateral luxation was pulpal necrosis (44.2%). Less frequent findings included surface resorption (14.0%), inflammatory resorption (8.5%), pulp canal obliteration (8.1%), and replacement resorption (0.9%). The included studies were cohort studies, which resulted in great heterogeneity, and the authors could not attempt a meta-analysis.Conclusions and Practical ImplicationsA large number of permanent teeth that experience lateral luxation are at risk of developing pulpal necrosis and other complications. Careful follow-up is required for these patients to treat complications as early as possible.
